The degree to which T-cell cross-reactivity actually immunizes individuals hasn’t been proven. If T-cell cross-reactivity provides meaningful immunity, it would offer significant progress toward herd immunity. The prevalence of cross-reactive immunity may vary substantially by region. The other variables will also have much to say about the timeline to reach herd immunity (see sidebar, “Key factors affecting the timeline to herd immunity”).
Tests that identify host immune response are referred to as serological tests and are intended to be administered as simple blood tests. These tests can indicate whether an individual has been previously exposed to COVID-19. Antibody test results are important in detecting infections in individuals with few or no symptoms and have been used in conjunction with RT-PCR results in establishing a diagnosis or exclusion of COVID-19 infection. IgM and IgG are the two major antibodies assessed by serologic testing.

During that time, our group was working on a study examining the performance of over-the-counter tests in the general population. We used data from this study to look at the performance of these tests both before and after omicron became the dominant variant in the United States. Our study, which has not yet been peer-reviewed, was unique because it was testing people for the COVID-19 virus over the course of two weeks, and thus we were able to observe emerging infections. Early in the omicron surge, around December 2021, people wondered about the ability of rapid tests to detect the new variant. Additionally, some preliminary tests proved that rapid tests that identify the omicron variant showed a one-to-two-day delay in a positive result compared to tests performed with the delta variant.
Although the second booster was safe and resulted in a large bump in neutralizing antibodies, vaccine efficacy was low against COVID infections. We are excited to be a part of the research and translation of this technology. Are recovered persons with persistent positive test of COVID-19 infectious to others?
Persons who have tested persistently or recurrently positive for SARS-CoV-2 RNA have, in some cases, had their signs and symptoms of COVID-19 improve. When viral isolation in tissue culture has been attempted in such persons in South Korea and the United States, live virus has not been isolated. There is no evidence to date that clinically recovered persons with persistent or recurrent detection of viral RNA have transmitted SARS-CoV-2 to others.
Despite these observations, it’s not possible to conclude that all persons with persistent or recurrent detection of SARS-CoV-2 RNA are no longer infectious. There is no firm evidence that the antibodies that develop in response to SARS-CoV-2 infection are protective. If these antibodies are protective, it’s not known what antibody levels are needed to protect against reinfection.
